미국에서 암과의 전쟁에서 놀라운 발전은 더 많은 어린이와 청소년, 성인들이 암 진단 후 더 오래 생존하고 있다는 사실에서 나타난다. 미국의 모든 암에 대한 5년 상대적 생존율은 1970년대 중반에 진단을 받은 사람의 49%에서 2010년부터 2016년까지 진단을 받은 사람들은 70%로 높아졌다. 2010년부터 2016년까지 미국 소아청소년(0~19세) 사이에서 진단을 받은 모든 암의 5년 상대 생존율은 85%로 1970년대 중반에 진단을 받은 환자의 생존율 63%에 비해 많이 상승했다.

미국에서 암에 대한 놀라운 진전이 이루어지고 있다는 또 다른 징후는 1991년 이후 연령 조정 전체 암 사망률이 감소하고 있다는 것이다. 미국 전체 암 사망률이 한 해만에 가장 큰 폭으로 감소한 것은 2016년부터 2017년까지 2.2%로 이들 데이터를 이용할 수 있는 마지막 해다. 1991년 최고치였던 이래로, 그 비율은 29퍼센트나 감소했고, 이것은 290만 명 이상의 암 사망 예상자의 목숨을 구했다는 것을 의미한다.

미국의 연령 조정 전체 암 사망률 감소는 주로 흡연율 감소에 따른 폐암 사망률의 급격한 감소에 의해 상당 부분 영향을 받고 있다. 폐암 사망률 감소는 최근 몇 년 사이 가속화돼 2008년부터 2013년까지 매년 2.4% 감소한 뒤 2013년부터 2017년까지 매년 4.3%씩 감소하고 있다. 2013~2017년 피부암 중 가장 치명적인 유형인 흑색종의 사망률도 매년 6.4%로 현저히 떨어졌다.

새로운 항암 치료법이 FDA의 승인을 받고 암의 예방과 생물학, 검출, 진단, 치료, 생존에 관한 과학적, 임상적, 정책적 진보를 거듭하는 만큼 암에 대한 진전의 속도를 가속화할 것이다. 이 보고서에서는 2019년 8월 1일부터 2020년 7월 31일까지 12개월 동안 이루어진 진전에 초점을 맞춘다. 이 기간 동안 발생한 암 치료의 진보에는 FDA가 승인한 20종류의 항암 치료법이 있다. 또한 이 기간 동안 FDA는 이전에 승인한 15가지 항암 치료제에 대해 새로운 암에도 추가하도록 승인 범위를 확대했다.

암에 대한 지속적인 공중보건의 도전

비록 우리가 암에 대해 놀라운 진전을 이루었지만, 이 질병은 전 세계적으로 계속해서 엄청난 공중보건(의료) 분야의 도전 과제이다. 2020년에 들어, 코로나바이러스 질환 2019(COVID-19)의 대유행으로 암에 대한 추가 진전은 더욱 복잡해졌다. 한 예로, COVID-19 대유행이 유방암과 대장암의 암 검사와 치료에 미치는 역효과로 인해 미국에서 향후 10년 동안 이 두 종류의 암으로 인한 사망자가 최소 1만 명 이상 더 있을 것으로 추정된다.

미국에서 암으로 인한 공중 보건 문제는 연구원들이 2020년에 1,806,590명의 새로운 암 환자가 발생할 것이며 606,520명의 사망자가 있을 것이라고 예상하고 있다는 사실에서 증명된다. 이 수치는 매일 매시간 206명의 새로운 암환자와 69명의 암 사망자가 나온다는 것을 의미한다.

                암, 세계적인 공중보건의 도전 과제

암은 전 세계적으로 질병과 사망의 주요 원인이다. 이러한 데이터를 이용할 수 있는 마지막 해인 2018년에 전세계 사망자의 16%를 차지했다.

전 세계적인 전체 암 발병부담

암의 엄청난 파괴적인 영향은 암 예방에 대한 새롭고 더 효과적인 접근법이 없는 한 앞으로 수십 년 안에 크게 증가할 것으로 예측된다.

조기 발견과 치료법을 개발하여 효과적으로 시행한다. 전체 암에 대한 전 세계 부담 증가의 예상치는 전체 인구 증가와 65세 이상의 암에 걸릴 가능성이 가장 높은 세계 인구 부문의 확대에 의해 크게 촉진될 것이다.

세계적인 소아암 발병 부담

2020년에는 0세에서 14세 사이의 41만 3천 명의 어린이가 암에 걸릴 것이고 32만 8천 명의 아이들이 질병으로 사망할 것으로 추정된다. 의료 접근성이 눈에 띄게 개선되지 않는 경우, 특히 중-저소득 국가 및 저소득 국가에서 그렇다. 2020년부터 2050년까지 총 1,370만 건의 소아암 발병과 1,110만 건의 소아암이 발생할 것으로 예상된다. 이 사례들과 사망자의 대부분은 중-저소득층 국가에서 생길 것이다. 의료 개입의 종합적인 확장을 가능하게 하는 투자는 2020년부터 2050년까지 어린이 암으로 인한 사망자 중 약 620만 명을 예방할 수 있는 잠재력을 가지고 있다.

암의 종류와 암 하위 유형 및 병기에 따라 진전이 가변적이다

우리가 직면하고 있는 도전들 중 하나는 암에 대한 진전이 모든 종류의 암에 대해 균일하지 않다는 것이다. 또한 특정 종류의 암은 모든 하위 유형과 병기에 따라 균일하지 않았다.

이러한 도전은 진단을 받은 암의 종류와 진단 암의 하위 유형, 병기에 따라 미국 환자들의 5년 상대적 생존율이 크게 다르다는 사실에서 잘 드러난다. 예를 들어 전립선암 남성의 경우 전체 5년 상대 생존율이 98%, 만성 림프구성 백혈병(CLL) 성인의 경우 85%, 간암 환자의 경우 18%, 급성 골수성 백혈병(AML)의 경우 25%로 전체 5년 상대 생존율이 극명한 대조를 이룬다. 유방암 여성 중 3중 음성 아형을 진단받은 여성은 5년 상대 생존율이 77%인 반면 호르몬 수용체 양성 아형을 진단받은 여성은 5년 상대 생존율이 90%를 넘는다.

폐암의 두 가지 주요 아형에 대해서도 5년 상대 생존율의 상당한 변화가 나타나고 있는데, 이는 비소세포폐암(NSCLC) 환자의 24%, 소 세포폐암 환자의 6%이다. 또 자궁내막 암에 걸린 여성과 대장암에 걸린 성인 중 암이 자궁에 국한된 경우, 또는 대장이나 직장에 국한된 경우 각각 5년 상대 생존율이 95%, 90%인 반면 암이 전이된 여성은 5년 상대 생존율이 17%, 14%인 것으로 나타났다.

더 많은 종류의 암의 조기 발견을 위한 새롭고 효과적인 검사법을 개발하는 것은 암이 신체의 다른 부분으로 전이되기 전에, 초기 단계에 있을 때 진단을 받은 환자들은 원격 부위로 전이 진단을 받은 환자들보다 장기 생존 가능성이 훨씬 높기 때문에 암의 유형들 간의 진전이 가변적이라는 과제(도전)를 해결하는 데 도움이 될 수 있다.

미국의 특정 인구 집단들 사이에 암 발병 부담에 대한 수많은 척도에 역차이가 존재한다. 최근에 확인된 암 발병률과 사망률 및 결과의 격차의 예는 여기에서 강조된다. 다른 암 대책의 격차는 보고서의 다른 곳에 설명되어 있다.

국립암연구소(NCI)는 암 건강 격차를 새로운 환자 수와 사망 건수, 암 관련 건강 합병증, 암 치료 후 생존과 삶의 질, 검진 비율, 특정 인구 집단 사이에 존재하는 진단 병기 등 암의 측정의 역차이로 정의한다.

암 건강 격차(불균형)는 많은 미국인들이 경험한다. (‘미국 인구 집단별로 어떤 암 건강 불균형을 경험하는가?’의 사이드-바 참조). 아프리카계 미국인은 오랫동안 불균형한 암 발병의 부담을 안고 있는 한 집단이다. 예를 들어 1993년 흑인 성인의 전체 암 사망률은 백인 성인의 사망률보다 33% 높았다. 고무적인 점은, 이러한 차이는 1993년부터 2017년까지 백인 성인보다 아프리카계 미국 성인들 사이에서 전체 암 사망률이 더 빠르게 감소했기 때문에, 이러한 데이터를 이용할 수 있는 마지막 해인 2017년까지 17퍼센트로 좁혀졌다. 아프리카계 미국인과 백인 사이의 결과의 차이를 없애기 위한 또 다른 진전의 신호는 2011년부터 2014년까지 아프리카계 미국인의 5년 암 생존율이 백인에 비해 더 많이 증가했다는 것이다. 그 결과, 흑인과 백인 간의 5년 암 생존율 차이는 8.2%에서 7.7%로 좁혀졌다. 이러한 진전에도 불구하고, 전반적인 암 사망률의 부담은 여전히 백인들에 비해 아프리카계 미국인들 사이에서 현저하게 높다.

아프리카계 미국인을 포함한 소수민족과 인종은 유달리 많은 암 부담을 짊어지고 있을 뿐만 아니라 현재 진행 중인 COVID-19 대유행의 많은 부담도 짊어지고 있어 의료의 극명한 격차를 더욱 부각시키고 있다. 의료의 불균형은 인종 불평등과 부당함의 가장 큰 형태 중 하나이며, 우리의 가장 기본적인 인권 중 하나인 건강 형평의 장벽인 사회적 불평등을 근절하는 데 모두가 역할을 해야 한다.

암 건강의 격차(불균형)를 포함한 건강 불균형의 원인을 식별, 수량화 및 이해하는 것은 이러한 불균형을 제거하기 위한 전략을 개발하고 실행하기 위한 필수적인 단계다. 암 건강 불균형에 기여하는 복잡하고 상호 관련 요인에 대한 현재 지식은 AACR 암 불균형 진행률 보고서 2020에서 자세히 논의된다(미국 암 건강 불균형이 존재하는 이유의 사이드-바 참조). 인종과 소수민족의 경우, 전부는 아닐지라도, 이러한 많은 요소들의 역 차이는 구조적이고 체계적인 인종차별주의의 직접적인 영향을 받는다. 모든 미국 인구 집단의 표본을 이용한 기초연구와 임상시험에 이 모든 집단의 개인들의 참여, 그리고 모든 이해 당사자들 간의 협력 증대를 통해 얻은 새로운 통찰력을 통해 우리는 암을 제거하는 데 큰 진전을 이룰 수 있을 것이다.

증가하는 암의 인구 부담

우리가 암의 예방과 조기 발견, 치료에 대한 개선된 전략을 개발하고 효과적으로 시행하지 않는 한, 암으로 인한 공중 보건 문제는 앞으로 수십 년 안에 미국과 전 세계에서 상당히 증가할 것이다.

미국에서는 2040년에는 새로운 암 발병 건수와 암 사망자가 각각 230만 명 이상, 100만 명에 육박할 것으로 예측된다. 현재의 수치에 비해 이러한 급격한 증가는 전체 인구 증가와 65세 이상 고령자 등 대부분의 암 진단을 받는 미국 인구의 부문이 2020년 5600만 명에서 2040년 8100만 명으로 증가할 것으로 예상되기 때문에 크게 기대되고 있다.

암은 주로 노화로 인한 병이다. 미국의 경우 진단 시 중위수 연령이 66세로 암환자의 54%가 65세 이상에서 진단을 받는다. 가장 최근의 자료에 따르면 2012년부터 2016년까지 매년 0.6%씩 감소하는 등 미국 전체 암 발병률을 줄이는 진전이 이뤄지고 있다. 그러나 49세 이하의 사람들 사이에서 일부 암의 발병률이 놀라운 속도로 증가하고 있다. 예를 들어 49세 이하 연령층의 대장암 발병률은 2012년부터 2016년까지 매년 2.2%씩 증가했다. 초기 대장암의 이러한 증가는 주로 비 히스패닉 백인들 사이의 대장암 발병률의 증가에 기인했다. 같은 기간 65세 이상 연령층의 대장암 발병률은 매년 3.3%씩 떨어졌다. 전립선암에서도 비슷한 추세가 나타났다. 두 사례 모두, 젊은 층은 암이 원격 부위로 전이되었을 때 진단을 받을 가능성이 더 높았고, 대장암 검진 지침으로 간주되는 데이터를 검토해보면 다음과 같다.

전문가의 합의를 거친 암 검진 권고사항

미국 정부와 많은 전문가 사회와 단체들은 가장 흔히 검진을 실시하는 5대 암에 대한 검진 검사의 사용에 대한 증거 기반 권고안을 가지고 있다. 여기서는 2020년 7월 31일 현재 미국 정부의 예방 서비스 태스크포스(USPSTF)와 미국 암 협회(ACS), 미국 종합 암 네트워크(NCCN), 미국의사협회(ACP), 미국 산부인과협회(ACOG), 미국 비뇨기과협회(AUA) 및 미국 다원 사회 특별위원회(MSTF)의 이러한 권고사항 중에서 합의를 강조한다. 모든 전문가 사회와 기관이 모든 암 검사에 대한 권고안을 갖고 있는 것은 아니다.

유방암 검진

평균 유방암 발병 위험이 있는 50~74세 여성은 정기적인 유방조영촬영 검사를 받아야 한다는 게 ACOG, ACP, ACS, USPSTF의 공통된 의견이다. 그러나 이 심사를 매년 해야 하는지 아니면 격년으로 해야 하는지에 대해서는 가변성이 있다.

일부 전문 사회와 단체들은 평균적으로 유방암에 걸릴 위험이 있는 여성들에게 40세에서 45세 사이에 정기적인 유방조영촬영 검사를 시작하라고 권고했다. 주목할 필요가 있다. 그러나 모든 집단이 이들 여성들에게 적합하다고 판단할 경우 정기적인 유방조영술을 받을 수 있는 기회를 갖는 것을 지지한다.

자궁경부암 검진

ACOG, ACP, USPSTF 사이에 다음과 같은 의견 일치가 있다.

▲21세 미만의 평균 위험 여성은 검진하지 않아야 한다.

▲21세에서 29세 사이의 평균 위험 여성은 3년마다 Pap test를 받아야 한다.

▲30세에서 65세 사이의 평균 위험 여성은 3년마다 Pap 테스트와 5년마다 Pap 테스트 및 HIV 테스트를 받거나, 혹은 매 5년 마다 HPV 검사 중 한 가지를 받아야 한다.

▲65세 이상의 여성은, 이전에 정상적인 결과를 가진 정기 검진을 받은 적이 있다면 달리 자궁경부암에 걸릴 위험이 높지 않기 때문에 평균적으로 질병에 걸릴 위험이 있는 경우에는 검진을 받지 않아야 한다.

ACS는 최근 평균적으로 자궁경부 위험이 있는 여성이 검진을 시작하는 나이를 21세에서 25세로 올릴 것을 권고했다.

전립선암

ACS, ACP, AUA, USPSTF는 전립선암 발병 위험이 평균적인 55~69세 남성들이 검진이 자신에게 적합한지 결정하기 전에 PSA 검사의 유익성과 잠재적인 위해성에 대해 의사와 상담한다는데 의견이 일치하고 있다.

대장암 검진

ACS, ACP, NCCN, USPSTF 사이에서는 대장암 발병 위험이 평균적인 50~75세 성인을 검진해야 한다는 공감대가 형성돼 있다. 어떤 사람이 얼마나 자주 검사를 받아야 하는지는 사용된 검사 검사에 따라 달라진다.

ACS 등 일부 전문 학회는 45세부터 정기검진을 시작할 것을 권고했고, 일부 전문 학회는 다른 전문 학회에 비해 특정 검진 방식을 권장하고 있다. 그러나 종합적인 메시지는 승인된 검진을 받는 것이 검진을 받지 않는 것 보다 좋고, 평균 정도 위험동의 성인은 검진을 언제부터 시작할지 자신에게 적합한 검진을 받기 위해서는 의료진과 상의해야 한다.

일부 집단의 개인은 대장암의 위험이 증가하고 있다. 예를 들면, 대장암의 검진 지침은 이런 집단에 다라 다양하지만, 모든 집단은 조기 및/혹은 더 잦게 가용한 검사를 받기를 권한다.

▲NCCN과 MSTP는 대장암에 대해 아래와 같이 권장한다. 린치 증후군을 일으키는 유전자 변이를 유전으로 받아 위험도가 증가한 사람은 20-25세 사이에 매 1-2년 마다 대장경 검사를 받아야 한다. 혹은 가족 중에 25세 이전에 발병한 사례가 있으면 그 발병의 2-5년 전에 받아야 한다.

▲ACS와 NCCN 및 MSTP는 대장암에 대해 다음과 같이 권고한다. 대장암 진단을 받은 사람의 최근친인 사람은 40세에 검진을 받거나 그 사람이 어려서 대장암 진단을 받았다면 그 10년 전에 검진을 시작하도록 권고한다.

USPDTF의 대장암 검진 지침은 현재 검토 중이다. 검토에 문제가 되고 있는 것은 모든 평균 위험도 사람들에 비해 이른 나이에 검진을 시작해야 하는지와 권장지침이 민족 및/혹은 인종에 따라 과연 다양한지 여부이다.

폐암

ACS와 NCCN 및 USPSTF 간에는 페암의 위험도가 높은 55-80세 성인에 대한 저 선량 검퓨터단층촬영(CT)을 통한 매년 검진을 제한해야 한다는 합의가 되어 있다. 왜냐하면, 30년 동안 하루에 한 갑의 담배를 피웠거나 혹은 15년간 하루에 2갑을 피웠고 현재 피우고 있거나 지난 15년 이내에 금연을 하였기 때문이다.

Research continues to be our best defense against cancer because it is the driving force behind all clinical and policy advances that improve cancer prevention, detection, diagnosis, treatment, and, increasingly, cures for individuals around the world.

Each advance that spurs progress against cancer is the result of many years of collaboration between different stakeholders dedicated to fundamentally changing the face of this devastating disease (see sidebar on Driving Progress against Cancer Together).

In the United States, the remarkable progress being made against cancer is illustrated by the fact that more children, adolescents, and adults are surviving longer after a cancer diagnosis. The U.S. 5-year relative survival rate for all cancers combined rose from 49 percent for people diagnosed in the mid-1970s to 70 percent for those diagnosed from 2010 to 2016. The 5-year relative survival rate for all cancers diagnosed among U.S. children and adolescents (ages 0–19) from 2010 to 2016 was 85 percent, up from 63 percent for those diagnosed in the mid-1970s. (see Figure 1).

Another sign of the extraordinary progress being made against cancer in the United States is that the age-adjusted overall cancer death rate has been declining since 1991. The largest reduction in the U.S. age-adjusted overall cancer death rate ever seen in a single year, 2.2 percent, occurred from 2016 to 2017, which is the last year for which these data are available. Overall, since its peak in 1991, the rate has declined by 29 percent, a reduction that translates into more than 2.9 million cancer deaths avoided.

The decline in the U.S. age-adjusted overall cancer death rate has been fueled in large part by a dramatic decline in the lung cancer death rate predominantly as a result of reduced smoking rates. The decline in the lung cancer death rate has accelerated in recent years, falling 2.4 percent each year from 2008 to 2013 and then falling 4.3 percent each year from 2013 to 2017. During the 2013 to 2017 period, the death rate for melanoma, which is the deadliest type of skin cancer, also fell at a remarkable rate of 6.4 percent each year. This striking reduction has been attributed in large part to the innovative new therapeutics approved by the U.S. Food and Drug Administration (FDA) for treating certain patients with the disease since 2011 (see Figure 2)

As more and more new anticancer therapeutics are approved by the FDA and we continue to make scientific, clinical, and policy advances in cancer prevention, etiology, detection, diagnosis, treatment, and survivorship, we will accelerate the pace of progress against cancer. In this report, we focus on advances made during the 12 months from August 1, 2019, to July 31, 2020. Among the advances in cancer treatment that occurred during this period are the 20 new anticancer therapeutics approved by the FDA for introduction into the clinic. In addition, during this period, the FDA expanded the uses of 15 previously approved anticancer therapeutics to include additional types of cancer (see Progress across the Spectrum of Cancer Treatment..

CANCER: AN ONGOING PUBLIC HEALTH CHALLENGE

Although we have made incredible progress against cancer, this disease continues to be an enormous public health challenge around the world (see sidebar on Cancer: A Global Public Health Challenge). In 2020, making further inroads against cancer has been further complicated by the Coronavirus Disease 2019 (COVID-19) pandemic, as discussed in COVID-19 and Cancer). As one example, it is estimated that there will be at least 10,000 additional deaths from breast cancer and colorectal cancer over the next decade in the United States as a result of the adverse effect of the COVID-19 pandemic on screening and treatment for these two types of cancer.

The public health challenge posed by cancer in the United States is illustrated by the fact that researchers project that there will be 1,806,590 new cases of cancer diagnosed in 2020 and that there will be 606,520 deaths from the disease. These numbers translate into 206 new cancer cases and 69 cancer deaths every hour of every day.

VARIABLE PROGRESS AMONG TYPES OF CANCER, SUBTYPES OF CANCER, AND STAGES OF DIAGNOSIS

Among the challenges we face is that progress against cancer has not been uniform for all types of cancer. Nor has it been uniform for all subtypes and stages of a given type of cancer.

These challenges are illustrated by the fact that the 5-year relative survival rates for U.S. patients vary widely depending on the type of cancer diagnosed, the subtype of the cancer diagnosed, and the stage at diagnosis. For example, the overall 5-year relative survival rates of 98 percent for men with prostate cancer and 85 percent for adults with chronic lymphocytic leukemia (CLL) stand in stark contrast to the overall 5-year relative survival rates of 18 percent for people with liver cancer and 25 percent for those with acute myeloid leukemia (AML). Among women with breast cancer, those diagnosed with the triple-negative subtype have a 5-year relative survival rate of 77 percent, while those with the hormone receptor-positive subtype have a 5-year relative survival rate of greater than 90 percent. Substantial variation in the 5-year relative survival rate is also seen for the two main subtypes of lung cancer; it is 24 percent among patients with non-small cell lung cancer (NSCLC) and 6 percent among those with small cell lung cancer. In addition, among women with endometrial cancer and adults with colorectal cancer, those whose cancer is confined to the uterus, or to the colon or rectum, have 5-year relative survival rates of 95 percent and 90 percent, respectively, while those whose cancer has metastasized have 5-year relative survival rates of 17 percent and 14 percent, respectively.

Developing new and effective tests for the early detection of more types of cancer could help address the challenge of variable progress between types of cancer because patients diagnosed when cancer is at an early stage, before it has spread to other parts of the body, have a much higher likelihood of long-term survival than those diagnosed when the disease has spread to distant sites, an occurrence known as metastasis.

U.S. CANCER HEALTH DISPARITIES

Adverse differences in numerous measures of cancer burden exist among certain population groups in the United States. some recently identified examples of disparities in cancer incidence, mortality, and outcome are highlighted here. Disparities in other cancer measures are outlined elsewhere in the report.

The National Cancer Institute (NCI) defines cancer health disparities as adverse differences in cancer measures such as number of new cases, number of deaths, cancer-related health complications, survivorship and quality of life after cancer treatment, screening rates, and stage at diagnosis that exist among certain population groups (see sidebar on U.S. Cancer Health Disparities).

Cancer health disparities are experienced by many segments of the U.S. population (see sidebar on Which U.S. Population Groups Experience Cancer Health Disparities?). The African American population is one group that has long shouldered a disproportionate burden of cancer. For example, in 1993, the overall cancer death rate for African American adults was 33 percent higher than it was for white adults. Encouragingly, this disparity had narrowed to 17 percent by 2017, the last year for which these data are available, because the overall cancer death rate decreased more rapidly among African American adults than it did among white adults from 1993 to 2017. Another sign of progress toward eliminating disparities in outcomes between African Americans and whites is that there was a greater increase in 5-year cancer survival for African Americans compared with whites from 2011 to 2014. As a result, the disparity in 5-year cancer survival for African Americans compared with whites narrowed from 8.2 percent to 7.7 percent during that period. Despite the progress, the burden of overall cancer mortality is still significantly higher among African Americans compared with whites.

WHICH U.S. POPULATION GROUPS EXPERIENCE CANCER HEALTH DISPARITIES?

According to the National Cancer Institute, cance rhealth disparities in the United States are adverse differences in cancer measures such as number of new cases, number of deaths, cancer-related health complications, survivorship and quality of life after cancer treatment, screening rates, and stage at diagnosis that exist among certain population groups including :

racial and ethnic minority groups

individuals who lack or have limited health insurance coverage.

immigrants;

individuals with disailities;

individuals of different ancestry

residents in certain geographic locations, including rural area;

refugees or asylum seekers;

adolescents and young adults and

individuals of low socioeconomic

status;

members of the lesbian, gay, bisexual. and transgender community;

refugees or asylum seekers;

the elderly

Racial and ethnic minorities, including African Americans, not only shoulder a disproportionate burden of cancer, but also are shouldering a disproportionate burden of the ongoing COVID-19 pandemic, further highlighting stark inequities in health care. Disparities in health care are among the most significant forms of racial inequality and injustice, and it is imperative that everyone play a role in eradicating the social injustices that are barriers to health equity, which is one of our most basic human rights.

Identifying, quantifying, and understanding the causes of health disparities, including cancer health disparities, is a vital step toward developing and implementing strategies to eliminate these disparities. Current knowledge of the complex and interrelated factors that contribute to cancer health disparities is discussed in detail in the AACR Cancer Disparities Progress Report 2020 (see sidebar on Why Do U.S. Cancer Health Disparities Exist?). For racial and ethnic minorities, adverse differences in many, if not all, of these factors are directly influenced by structural and systemic racism. New insights obtained through research, including basic research using samples from all U.S. population groups, through the participation of individuals from all these groups in clinical trials, and through increased collaboration among all stakeholders will allow us to make major strides toward eliminating cancer for all.

CANCER: A COSTLY DISEASE

RESEARCH: A VITAL INVESTMENT

The enormous toll of cancer is felt not only through the number of lives it affects each year, but also through its immense economic impact.

In the United States, it is estimated that cancer health care spending was $161.2 billion in 2017, the last year for which these data are available. This does not include the indirect costs of lost productivity due to cancer-related morbidity and death, which were $30.3 billion and $150.7 billion, respectively. Overall, these numbers translate to about 1.8 percent of the country’s gross domestic product.

The economic burden of cancer stands in stark contrast to the amount of money the federal government invests across all areas of medical research. In 2017, the same year that cancer health care spending was $161.2 billion, the budget for the National Institutes of Health (NIH), which is the largest medical research agency in the world, was just $34.15 billion, of which $5.64 billion went to the NCI.

If the number of cancer cases diagnosed each year in the United States increases in the coming decades as anticipated, the direct and indirect costs will also escalate.

The increasing personal and economic burden of cancer highlights the vital need for more transformative research to accelerate the pace of progress. Recent advances, some of which are highlighted in this report, were made as a result of the cumulative efforts of researchers from a diverse array of specialties. Their work is supported in large part by funds from the federal government that are administered through the NIH. The consecutive multibillion dollar increases for the NIH budget from fiscal year (FY) 2016 to FY2020 have helped researchers keep up with the pace of scientific innovation (see Figure 21). It is imperative, however, that Congress continue to provide sustained, robust, and predictable increases in investments in the NIH and the NCI, as well as other federal agencies that are vital for fueling progress against cancer such as the FDA and the Centers for Disease Control and Prevention (CDC), in the years ahead (see The AACR Call to Action.)
