국립암연구소에 따르면 인공지능(AI)은 학습과 추리, 문제해결, 의사결정 등 보통 지능적인 인간의 행동이라고 생각되는 기능을 컴퓨터가 수행할 수 있는 능력으로 정의된다. 연구진이 스캔과 병리학 슬라이드, 암과 환자 게놈 프로필, 전자 건강 기록부터 임상 결과까지 암 관련 데이터를 대량 축적하면서 AI가 이 정보를 분석해 이전에는 실현할 수 없었던 의미 있는 통찰력을 도출할 수 있다. ‘머신러닝(machine learning)’은 인간의 명시적 개입 없이 데이터에 접근해 학습하고 패턴을 파악하며 의사결정을 내릴 수 있는 컴퓨터 프로그램 개발에 집중하는 AI의 응용 프로그램이다. ‘딥러닝(Deep learning)’은 신경망을 활용해 의사결정을 하는 머신러닝의 서브셋이다. 암 과학과 의학에서 AI의 적용은 방대하고 빠르게 확대되고 있다. 그 분야의 최근의 발전은 아래에 설명되어 있다.
액체 생검(조직검사)을 이용한 침습 최소화 시험
조직검사 혹은 생체검사(생검)는 의사가 암과 같은 질환을 진단하거나 치료에 반응하여 어떻게 변하는지 관찰할 수 있도록 검사용으로 환자로부터 세포나 조직을 떼어내는 것이다. 전통적으로 생체검사는 침습적(외과적) 시술이다. 그러나 암 발병과 치료 과정에서 종양은 일상적으로 발견 가능한 세포와 지질의 캡슐화된 주머니인 엑소좀과 그리고 자유로이 움직이는 DNA를 환자의 혈액이나 뇌척수 액으로 흘려보낸다는 연구 결과가 나왔다. 또한 최근 연구에서는 혈액이나 또 다른 생물 체액 시료, 즉 기존의 조직 생체검사가 아닌 '액체 조직검사'를 사용하여 환자의 암과 관련된 분자변화와 같은 귀중한 정보를 제공받기 위해 분석할 수 있는 물질을 얻을 수 있다는 것이 밝혀졌다. 따라서 액체 생검은 암의 상태를 탐지하거나 추적할 수 있는 덜 침습적인 수단을 제공한다. 암 분야에서는 종양 특성에 대한 스냅 샷만 제공하는 기존의 생체실험과는 달리, 액체 생검 접근법은 질병의 진행과 치료에 대한 반응의 모니터링을 허용함으로써 개인의 암에 대한 보다 완벽한 그림을 만들어낼 수 있다는 흥분감이 크다.
현재 진행 중인 연구는 암의 검진과 조기 발견, 치료 및 치료 반응의 모니터링의 미래 지형을 바꿀 수 있는 복수의 액체 생검 접근법을 평가하고 있다. 어떤 액체 생검 플랫폼은 혈액 샘플을 분석하여 특정 암 유형과 연관된 DNA의 특정 유전적 또는 후생 유전적 변화를 식별하는 반면, 다른 플랫폼은 혈액 속에 있는 분열된 세포 없는 DNA의 분열 양상을 보다 광범위하게 관찰하며, 다른 것들은 혈액 속에서 종양과 관련된 단백질을 검출하는 것을 목표로 한다. 초기 임상 데이터는 액체 생검이 질병과 치료 반응, 내성, 재발의 표지를 확인함으로써 암의 조기 발견과 차단, 진단, 치료, 감시를 변화시킬 수 있는 잠재력을 가지고 있음을 나타낸다. 암 연구의 스펙트럼에 걸친 액체 생물학의 역할을 조사하는 최근 연구의 선별된 사례가 여기에 제시되어 있다.
암의 조기 발견
최근의 한 연구에서, 연구원들은 혈액 검사를 영상 기법과 결합하여 어떠한 이전 기록이나 증상 없이 여성의 암을 감지할 수 있었다. 검사 결과 권장 선별검사가 있는 유방암, 폐암, 대장암 등이 확인됐지만, 현재 검진을 받을 수 없는 난소암, 자궁암, 신장암 등 7종의 추가 암도 확인됐다. 특히 일부 암은 초기 단계에서 발견되었는데, 이때 개입이 가장 효과적일 가능성이 높다. 두 번째 액체 생검 플랫폼은 혁신적인 DNA 염기서열 분석 방법을 사용하여 순환 DNA의 특정 패턴을 분석하여 50종 이상의 암을 검출했다. 이 플랫폼의 또 다른 이점은 암이 발생한 조직을 식별할 수 있었다는 것이다. 이러한 보고가 매우 유망하지만, 이러한 검사를 클리닉에 도입하기 전에 액체 생검을 이용한 암 검진이 궁극적으로 암으로 인한 사망자의 수를 줄일 수 있는지 여부를 결정하기 위한 추가적인 연구가 필요하다. 또한, 개인의 평생 동안 심각한 해를 끼치지 않을 느리게 성장하는 암을 감지하는 잠재적 해악, 즉 과잉 진단이라고 알려진 현상, 그리고 과잉 치료라고 알려진 불필요한 침습적 개입을 측정할 필요가 있다.
치료 의사 결정
분자 표적 치료법은 폐암이나 유방암과 같은 많은 질병에 대한 암 치료의 지형을 변화시켰지만, 치료의 대상이 될 수 있는 암의 모든 유전적 변형을 검사하기 위해 전통적인 생체 검사를 사용하는 것은 종종 어렵다. 근본적인 이유는 적절한 바이오 시료의 부족이나 또는 품질이 좋은 바이오 시료의 부족에서부터 준수의 문제 및 생물 윤리적 문제에 이르기까지 다양할 수 있다. 이런 점에서 액체 생검은 훌륭한 대안을 제공한다. 그것들은 잠재적으로 환자에게 더 안전하고 덜 침습적이며 일반적인 조직검사보다 종양 이질성을 더 잘 나타낼 수 있다. 따라서 이 기술의 가장 큰 매력 중 하나이자 FDA가 승인한 유일한 사용 허가는 암 유발 돌연변이를 식별하여 환자에 대한 치료 결정을 내리는 동반 진단이다. 지금까지 두 차례의 액체 생검 동반 진단 테스트가 FDA의 승인을 받았다. 2016년 6월 FDA는 전이성 비소세포 페암 환자가 EGFR(상피세포성장인자수용체) 표적 치료제 엘로티닙으로 치료를 받을 수 있는지 여부를 확인하는 것을 최초로 승인했다. FDA는 2019년 5월 HER2 음성, 재발 또는 전이성 유방암에 걸린 개인에서 PIK3CA 돌연변이를 검출하는 2차 액체 생검 동반 진단 테스트를 승인했다. 암환자의 혈액 또는 기타 생체유체 샘플을 사용하여 동시에 여러 표적 가능한 유전자 변형을 검출할 수 있는 수많은 새로운 액체 생검 플랫폼을 개발하고 검증하기 위한 연구가 진행 중이다. 이러한 테스트가 기존 생체검사와 동일한 정확도로 치료 지향적 돌연변이를 검출할 수 있는지, 액체 생검에서 파생된 정보에 기초한 치료법이 암환자에게 유사한 장기적 결과를 초래하는지 여부는 아직 결정되지 않았다.
암의 내성 및 재발 예측
액상 생검은 또한 연구자들에게 환자의 암이 퍼질 가능성이 있는지, 치료에 내성이 있는지 또는 재발할 가능성이 있는지에 대한 중요한 단서를 제공할 수 있다. 예를 들어 위암 환자의 ctDNA 분석을 통해 HER2 단백질을 발현하고 HER2 표적 치료 트라스투주맙으로 치료한 결과, 연구자들은 표적 치료제에 대한 내성에 기여하는 유전적 변화에 대한 새로운 통찰력을 얻을 수 있었다. 또 다른 최근 보고에 따르면, 초기 비소세포폐암 수술 중 검출된 세포 내에서 순환 종양 세포의 유병률과 특정 유전적 변화가 존재한다면 전이 암의 재발을 예측할 수 있을 것이다. 수술 후 또는 보조 항암화학요법 후 순환종양 DNA가 검출된 대장암에서도 3년 후 추적 관찰 과정에서 질병 재발의 초과 위험과 관련이 있는 유사한 데이터가 등장했다. 또한 3중 음성 유방암으로 알려진 특히 난치성 유방암의 재발 위험을 결정하기 위해 순환 종양 DNA 검출의 임상적 효용을 평가하기 위한 연구가 진행 중이다.

MINIMALLY INVASIVE TESTING USING LIQUID BIOPSIES
A biopsy is the removal of cells or tissues from a patient for testing to help physicians diagnose a condition such as cancer or monitor how it changes in response to treatment. Traditionally, biopsies are invasive procedures. However, research has shown that during cancer development and treatment, tumors routinely shed detectable cells, lipid encapsulated sacs called exosomes, and free DNA into a patient’s blood or cerebrospinal fluid. Recent studies have also shown that it is possible to use a blood or another biofluid sample, or “liquid biopsy,” rather than a traditional tissue biopsy, to obtain material that can be analyzed to provide valuable information such as the molecular alterations associated with a patient’s cancer. Liquid biopsies, therefore, provide a less invasive means to detect or track the status of cancer. There is much excitement in the cancer field that, as opposed to traditional biopsies which only provide a snapshot of the tumor characteristics at one specific timepoint, liquid biopsy approaches may generate a more complete picture of an individual’s cancer by allowing for the monitoring of disease progression and its response to treatments in real time.
Ongoing research is evaluating multiple liquid biopsy approaches that may transform the future landscape of cancer screening, early detection, treatment, and monitoring of treatment responses (see Figure 20). Some liquid biopsy platforms analyze blood samples to identify specific genetic or epigenetic alterations in the DNA that are associated with certain cancer types, while others look more broadly at the patterns of fragmentation of the shredded cell-free DNA in the blood, and yet others aim to detect tumor-associated proteins in the blood. Early clinical data indicate that liquid biopsies have the potential to transform early detection, interception, diagnosis, treatment, and surveillance of cancer by identifying markers of disease, therapeutic response, resistance, and recurrence. Selected examples of recent research examining the role of liquid biopsies across the spectrum of cancer research are presented here.
DETECTING CANCERS EARLY
In a recent study, researchers were able to utilize a blood test combined with imaging techniques to detect cancers in women without any prior history or symptom. The test identified breast, lung, and colorectal cancers for which there are recommended screening tests, but also seven additional cancer types such as ovarian, uterine, and kidney cancer for which there are no screening tests available at the current time. Notably, some of the cancers were detected at an early stage, when interventions are most likely to be effective. A second liquid biopsy platform utilized innovative DNA sequencing methods to analyze specific patterns in the circulating DNA to detect more than 50 different types of cancer. An added benefit of this platform was that it was able to identify the tissues in which the cancer originated. While these reports are very promising, additional research is needed to determine whether cancer detection using liquid biopsies can ultimately reduce the number of deaths from cancer, before such tests can be introduced to the clinic. Furthermore, potential harms of detecting slow-growing cancers that would have never caused serious harms during an individual’s lifetime, a phenomenon known as overdiagnosis, and of unnecessary invasive interventions known as overtreatment, need to be weighed against potential benefits.
MAKING TREATMENT DECISIONS
While molecularly targeted therapies have transformed the landscape of cancer treatment for many diseases such as lung or breast cancer, it is often difficult using traditional biopsies to test for all genetic alterations in the cancer that may be therapeutically targetable. Underlying reasons may vary ranging from lack of adequate biospecimen or lack of quality biospecimen, to compliance and bioethical issues. In this regard liquid biopsies provide a great alternative. They are potentially safer and less invasive for the patient and may better represent the tumor heterogeneity than a typical biopsy. Therefore, one of the biggest appeals and the only FDA-approved use of this technology is as a companion diagnostic in identifying cancer-causing mutations to make treatment decisions for patients. Thus far, two liquid biopsy companion diagnostic tests have been approved by the FDA. In June 2016, the FDA approved the first for identifying whether a patient with metastatic NSCLC is eligible for treatment with the EGFR-targeted therapeutic erlotinib. In May 2019, the FDA approved the second liquid biopsy companion diagnostic test which detects PIK3CA mutations in individuals with HER2-negative, advanced or metastatic breast cancer. Ongoing research is underway to develop and validate numerous new liquid biopsy platforms that can simultaneously detect multiple targetable genetic alterations using blood or other biofluid samples from patients with cancer. It remains to be determined whether such tests can detect therapeutically targetable mutations with the same accuracy as traditional biopsies and whether treatments based on liquid biopsy-derived information result in comparable long-term outcomes for patients with cancer.
PREDICTING CANCER RESISTANCE AND RECURRENCE
Liquid biopsies may also provide researchers with important clues as to whether a patient’s cancer has the potential to spread, grow resistant to treatments, or relapse. For instance, ctDNA analysis in patients with gastric cancer expressing the protein HER2 and treated with the HER2-targeted therapeutic trastuzumab allowed researchers to gain novel insights into the genetic alterations that contribute to resistance to the targeted therapeutic. According to another recent report, the prevalence of circulating tumor cells and the presence of certain genetic alterations within those cells detected during surgery of early-stage NSCLCs could predict the recurrence of metastatic cancer. Similar data have emerged in colon cancer where the detection of circulating tumor DNA after surgery or after adjuvant chemotherapy was associated with excess risk for disease recurrence during a 3-year follow-up. In addition, ongoing research is underway to evaluate the clinical utility of circulating tumor DNA detection in determining the risk of recurrence of a particularly intractable form of breast cancer known as triple-negative breast cancer.
